New Patient Information

We are excited to have you join us as a new patient at Live Oak Family Health. To help you get the most out of your visit:Bring your current medications with you. Also bring prescription creams, birth control pills, eye drops, over-the-counter medications, herbal/natural medicines, and vitamins. By knowing the exact details of your medications and supplements we will be able to take better care of you.

Requesting copies of your medical records

If you’ve had significant medical problems it will help your doctor if we can get a copy of your medical records from your previous provider. This can help prevent duplication of tests that you’ve already done. It can also help us avoid treatments that haven’t worked for you in the past. Please fill out the Live Oak Family Health PA Medical Records Release form included in your patient packet. You can give that to your previous provider and they will send the records to us. We recommend checking back with your previous provider after a week or so to make sure they’ve sent the records over to us. Or you can bring them to us yourself.

Arrival time

If you don’t complete the forms in advance please arrive 30 minutes early on your first visit so you have time to do this before your appointment time. If you have filled out the paperwork arriving 15 minutes early will help make sure you can be seen in a timely fashion.

Bring your insurance card

Be sure to bring your current insurance card with you to all of your appointments. We will verify your insurance status to make sure your insurance is active when you check in for your visit. If your insurance is no longer valid you may pay the prompt pay rate.
